You are covered and your case is being managed by WorkCover

Your treatment is covered by Veterans Affairs

You are claiming a rebate through the Chronic Disease Management Plan (CDM) formerly known as an EPC (Enhanced Primary Care Plan)

We may recommend you see your GP at a later stage for pain management and/or a referral to a specialist.

If you find yourself with pain or an injury that is impacting your day-to-day life, then it’s probably time to come and see a physiotherapist. Physiotherapy is suitable for people of all ages with a wide range of health conditions including problems affecting the bones, joints and soft tissue – such as back pain, neck pain, shoulder pain and sports injuries.

Yes. If you have private health cover and have “extras” which covers physiotherapy/myotherapy, you should be able to claim part of your treatment fee. This rebate will depend on your level of cover and your chosen health fund. For most health fund claims we are able to claim your rebate at the clinic leaving you to pay the gap between this payment and the fee. You will need to have your health fund card with you to make this claim.

In addition, Medicare offers the CDMP Chronic Disease Management Plan (CDM) formerly known as an EPC (Enhanced Primary Care Plan) which entitles people with a chronic condition to access a Medicare rebate on up to five physiotherapy sessions per year. A referral for this program must be come from a GP and a copy of this referral will need to be provided to the Clinic. The claim for this rebate can also be made at the clinic.

A Chronic Disease Management Plan (CDM) (formerly known as an EPC (Enhanced Primary Care Plan)) is a referral that is written by your GP which provides eligible patients with Medicare-subsidised allied health appointments. The rebate for Physiotherapy is usually $58.30, however if you have exceeded your Medicare safety net, it may be higher.

No. WorkCover and TAC fee schedules have been severely out of step with the fee structures of quality Victorian Private Practitioners for over 20 years.

The fee schedule of these authorities does not match the costs involved in running a quality practice with the scope of services provided, state of the art technologies and experienced physiotherapists and staff at these significantly lower fees.

Patients with a WorkCover or TAC claim would need to pay the full fee and manage the claim themselves.

We understand that circumstances may arise where you need to cancel or reschedule your appointment. We kindly ask for as much notice as possible if you need to change or cancel your appointment.

Please contact the clinic on (03) 8652 9158 to let us know of any cancellations

Failure to notify the clinic of your inability to attend an appointment could incur a cancellation fee equivalent to 50% of the initial appointment charge.
